Features of the organization and provision of palliative and hospice care in the context of the COVID-19 pandemic

Background. The purpose was to determine the main tasks and features of the organization and provision of palliative and hospice care (PHC) in a COVID-19 pandemic. Materials and methods. The materials of the study were national and international literature sources (7 units), medical statistics data (2 units) and international documents (7 units). We used bibliosemantic and statistical research methods, methods of system approach and content analysis were used in the work. Research. It is established that the urgency of the problem of organization and provision of PHC to the population in a COVID-19 pandemic is determined by the high level of morbidity and mortality of patients with COVID-19. Elderly and chronic diseases patients are in the risk for death from COVID-19. It is important to understand that older people have the same rights as others to receive affordable and high-quality health care, including intensive care and PHC. Patients with senile dementia can put themselves and others at risk of COVID-19 infection if they forget to follow hygienic and quarantine measures. Under quarantine conditions, it is necessary to ensure their communication with relatives and friends with the help of modern digital and telecommunication technologies. When providing PHC to palliative patients including COVID-19, in PHC facilities and at home, it is necessary to provide palliative patients and staff with personal protective equipment and to organize multidisciplinary cooperation. WHO guidelines call for the active detection of COVID-19 among patients, staff, and visitors in PHC facilities in accordance with approved clinical protocols. Given the urgency of the problem, the staff of the Department of Palliative and Hospice Medicine of the Institute of Family Medicine of the Shupyk NMAPE included relevant materials in lectures and seminars. PCBs should be part of national and international COVID-19 pandemic response plans. The Conclusions. Ministry of Health of Ukraine, regional health authorities need to develop and approve guidelines and clinical protocols for affordable, high-quality and effective PHC in the context of the COVID-19 pandemic, which provide a set of measures to: provide palliative patients and staff of PHC facilities and PHC field teams with personal protective equipment; ensure access to palliative patients including with COVID-19, to adequate analgesia, including the use of narcotic analgesics, and symptomatic treatment; greater use of modern means of communication, including telemedicine and video-communication; to ensure optimal professional training of medical and social staff, relatives and volunteers on the organization and provision of PHC in the context of the COVID-19 pandemic.Downloads
